1) Visit the CDC by clicking this link. Then go halfway down the page and you will see the tool where you can actually enter your information to see if you're eligible: https://www.cdc.gov/coronavirus/2019-ncov/vaccines/booster-shot.html
2) Visit the Tennessee state webpage. I chose the Tennessee state site because they have a great calculator that is very straightforward to use. Click the link here to visit their page: https://covid19.tn.gov/booster-calculator/. (Also check your own state's website to see if they have a similar tool).
3) Check out the FDA site which has a nice visual component that might be helpful:
